About the Role We are looking for a Telehealth Nurse to provide support to patients, caregivers, healthcare providers, and their teams through phone and electronic communication. The nurse will help patients with medication adherence, follow-ups, clinical information collection, education, and coordination of services while following applicable nursing, regulatory, and program requirements. Telehealth experience is preferred. Provide support to patients, caregivers, and healthcare professionals through phone and electronic communication. Use motivational interviewing and behavior modification techniques to encourage medication adherence. Conduct patient outreach and follow up according to program requirements. Collect and document patient-specific clinical information accurately. Identify patient needs, risks, and safety concerns and escalates issues when appropriate. Communicate with patients and healthcare providers regarding treatment compliance and follow-up. Follow all applicable healthcare, regulatory, and program requirements. Identify and appropriately report adverse events and product complaints. Maintain active and current nursing licensure throughout employment. Required Qualifications RN or LPN/LVN license is required. Current and active nursing license must be maintained throughout employment. At least 2 years of healthcare experience in a healthcare organization, physician office, laboratory, or similar setting. Vocational or technical nursing training/education. Strong communication and documentation skills. Knowledge of medical terminology and basic mathematics. Good computer and data-entry skills. Working knowledge of Microsoft Office/Microsoft Suite. Ability to communicate professionally with patients, caregivers, and healthcare professionals. Strong problem-solving and independent decision-making skills. Ability to research services and resources to assist patients. Excellent attention to detail and accuracy when collecting and documenting information. Comfortable working with multiple online platforms and applications at the same time. Ability to develop action plans and help resolve complex patient-related issues. Adaptable and comfortable working in a fast-paced healthcare environment. Strong organizational and time-management skills. Work From Home Requirements Must have a dedicated, professional, HIPAA-compliant workspace . Reliable high-speed internet is required to support multiple applications, phone/VOIP calls, and video conferencing. Appropriate childcare arrangements are required during working hours. This is a call-center-style remote environment , so a quiet and professional workspace is expected. Remote work from home. May sit or stand approximately 7-10 hours per day . Extended hours may occasionally be required based on business needs. Headset use may be required for extended periods.
